Once you open the link you will get the page like this

Click the New Order

After clicking "New Order", you will be presented with an order form. Please fill in the following fields:

Account ID: 9489

Surgery Date: Today's date

Order ID: Enter a unique order identifier.

Mode of Order: Select "FAX"

NPI: Enter any three numbers to view options, then select one.

Capital Equip. Used: Select "No EGPS"

In cart detail: [Provide specific instructions for this field]

Part No:  i. Part No: 650.010 and other available parts. Select a different part number for each row

Order Qty:  3(Enter the desired quantity. The 'shelf quantity' shows how many parts are available. If it's zero, please select a different part number)

Type: select “C”

After filling out the form, scroll down to find the "Place Order" button. It will move to next page from there just click the Print Slip

After clicking the print Slip save the Print Slip as pdf.
